Veterans Bonus Program - Eligibility Requirements

Your application will not be processed for compensation until we have received your signed, acknowledged or notarized application and the required documents indicated above.

Eligibility Criteria

A person who served or is serving, greater than 90 days, on active duty service in the US Armed Forces, excluding dates for training, during at least one of the compensable time periods and meets requirements may be eligible for the Ohio Veterans Bonus.

  1. Served or currently serving in the US Armed Forces during compensable periods:
    1. Afghanistan Oct 7, 2001 through a date to be determined by President of the United States.
    2. Iraq Mar 19, 2003 through Dec 31, 2011 (DEADLINE to apply Dec 31, 2014).
  2. The person was separated from the United States armed forces under honorable conditions, is still serving in active duty or after serving on active duty remains in any reserve component of the United States armed forces or in the Ohio National Guard.
  3. The person was an Ohio resident at the start of active duty service.
  4. The person is currently an Ohio resident.
  5. The person has not received a bonus or compensation of a similar nature from another state.

Compensation

Eligibility and compensation for the Ohio Veterans Bonus program is based upon where and when the veteran or service member served during active duty. The following calculations are applied in determining bonus awards:

  • $100 per month, not to exceed $1,000, for service in Afghanistan or Iraq during the compensable periods.
  • $50 per month, not to exceed $500, for service anywhere else in the world during the compensable periods.

Service for a fraction of a month shall be paid at the rate of 1/30 of the appropriate monthly amount for each day of service. Compensation may not exceed $1,500 for combined service.

Active Duty Service Member

US Armed Forces active duty service members who have or are serving greater than 90 days, excluding training, may be eligible for bonus compensation if they meet the eligibility criteria (1-5) indicated above.

Required Documentation

  • Military records showing dates served in Afghanistan and/or Iraq (with appropriate medals awarded for combat service, such as the Afghanistan Campaign Medal, or Iraq Campaign Medal) or for service anywhere else in the world.
    • Army: Officer Record Brief (ORB) or Enlisted Record Brief (ERB).
    • Air Force: Virtual Military Personnel Flight (MPF), First page of Enlistment Contract (Form 4/1), Page showing medal racks.
    • Navy: First page of Enlistment Contract (Form 4/1), Enlisted Service Record (ESR).
    • Marines: Basic Individual Record (BIR) pages 1-3.
  • The following document may be used to establish active duty service and Ohio residency:
    • Leave and Earning Statement (LES) (most recent at the time of application).

Veteran

Former members of the US Armed Forces (Veterans) who served greater than 90 days, excluding training, may be eligible for bonus compensation if they meet the eligibility criteria (1-5) indicated above.

Required Documentation

  • DD-214 that includes blocks 23-30 (will provide home of record at start of military service, dates served in Afghanistan and/or Iraq or anywhere else in the world, character of service at discharge, and time lost.)
  • Proof of Ohio residency at start of military service if DD-214 shows other than Ohio as "home of record".
  • Proof of current Ohio residency
  • The following documents may be used to establish Ohio residency:
    • Valid Ohio driver’s license
    • Current Utility Bill
    • Current Pay Stub
    • Lease agreement or Mortgage statement
    • Ohio State Income Tax Return - should be Ohio tax records from time you entered active duty service, or to prove current residency, provide Ohio tax records as of the date application for Veterans Bonus signed and notarized.

Combat-related medical discharge or medically retired from service as a result of injury or illness resulting from service in Afghanistan or Iraq

Medically discharged or retired veterans who meet all eligibility criteria above may be eligible for an in-country bonus of $1,000 (regardless of time served in-country), and may be eligible for anywhere else in the world service at $50 per month, excluding training time. The maximum bonus award is $500 for anywhere else in the world service. A medically discharged or medically retired veteran may be eligible for a maximum bonus of $1,500.

Required Documentation

  • DD-214 that includes blocks 23-30 (will provide home of record at start of military service, dates served in Afghanistan and/or Iraq or anywhere else in the world, character of service at discharge and time lost.)
  • Proof of current Ohio residency
  • The following documents may be used to establish Ohio residency:
    • Valid Ohio driver’s license
    • Leave & Earnings Statement (most recent as of date of application)
    • Voter registration records
    • Ohio State Income Tax Return - should be Ohio tax records from time you entered active duty service, or to prove current residency, provide Ohio tax records as of the date application for Veterans Bonus signed and notarized.
  • Physical Evaluation Board (PEB) Decision

Family of an eligible veteran whose death was not a result of injuries or illness sustained in Afghanistan or Iraq service

The family of an eligible veteran whose death was not a result of injuries or illness sustained in Afghanistan or Iraq service, may be entitled for the bonus the veteran would have received if he/she were living in the following order of precedence: spouse, child or parents of veteran, including a person or persons serving as legal guardian for at least one year preceding commencement of service in the United States armed forces. Applicant must ensure that deceased service member met eligibility criteria note in 1-5 above.

Required Documentation

  • Casualty Report (DD 1300), if service member was on active duty at time of death, or
  • DD-214 that includes blocks 23-30 (only if veteran was discharged from military at time of death), (will provide home of record at start of military service, dates served in Afghanistan and/or Iraq or anywhere else in the world, and character of service at discharge.), or
  • Proof of Veteran's Ohio residency at time of death.
  • Proof of relationship to veteran:
    • Marriage certificate (spouse)
    • Birth certificate or adoption records (children)
    • Veteran birth certificate or adoption records (parents)

  • Death certificate (If you do not have a death certificate call 1-877-OHIO VET (1-877-644-6838) to speak with a member of bonus team.)

Family of an eligible veteran whose death occurred as a result of injuries or illness sustained in Afghanistan or Iraq

The family of an eligible veteran whose death occurred as a result of injuries or illness sustained in Afghanistan or Iraq service, may apply for the bonus in the following order of precedence: spouse, child or parents, including a person or persons serving as legal guardian for at least one year preceding commencement of service in the United States armed forces, may be entitled to compensation of $5,000. To be eligible, the service member must have met eligibility criteria (1-5) stated above. Family members are not required to reside in Ohio.

Required Documentation

  • Casualty Report DD-1300 (provides proof of residency at time of military entry, date entered active duty and date of death).
  • Proof of service member's Ohio residency at time of death.
  • If service member died following evacuation from in-country provide Physical Evaluation Board (PEB) decision or Veterans Affairs Rating Decision (includes language regarding disability and location where injury/illness occurred).
  • Additional records demonstrating service in-country (Afghanistan or Iraq) may be necessary (call 1-877-644-6838 to speak with a member of bonus team).
  • Proof of relationship to veteran:
    • Marriage certificate (spouse)
    • Birth certificate or adoption records (children)
    • Veteran birth certificate or adoption records (parents)

Veteran or family of a Service member or Veteran who was declared Prisoner of War or Missing in Action

A service member or veteran who was declared Missing in Action or Prisoner of War or the family of a POW/MIA (in order of precedence: spouse, child or parents) who meets eligibility criteria (1-5) stated above, may be eligible for $5,000.

Required Documentation

  • Proof of service member or Veteran’s Ohio residency at start of military service.
  • Proof of service member or Veteran’s Ohio residency at time of DoD Declaration.
  • Department of Defense Declaration of Prisoner of War or Missing in Action.
  • Veterans’ Affairs Rating Decision.
  • Proof of relationship to veteran:
    • Marriage certificate (spouse)
    • Birth certificate or adoption records (children)
    • Veteran birth certificate or adoption records (parents)

All applicants will be required to complete either an on-line application or a printed application.
